Methods Of Detection Of Coronary Artery Disease

Exercise electrocardiography, the least expensive noninvasive test for myocardial ischemia is two times less expensive than stress echocardiography and five times less expensive than stress single-photon-emission computed tomography (SPECT). Drugs such as digoxin may cause false positive results in the exercise electrocardiogram (138). Data pooled from 132 studies show that the sensitivity and specificity of routine exercise electrocardiography are 68 and 77 , respectively (139,140). However, there is wide variability in the studies, depending upon the diagnostic criteria used, and the patient population. For example, women have increased false positive rates on routine treadmill testing as compared with men. Inability to achieve 85 of maximum predicted heart rate may influence the results of the test. Negative inotropic medications such as beta blockers and calcium channel blockers may decrease the sensitivity of the test, resulting in false negative tests.

Exercise Testing in Patients with Symptoms or a Prior History of CAD

Generally, patients with suspected or known CAD and new or changing symptoms that suggest ischemia should undergo exercise testing to assess the risk of future cardiac events, unless cardiac catheterization is indicated. The probability that the coronary disease of a given patient will progress to a higher risk state depends on multiple factors related to the underlying atherosclerotic process. The pathophysiologic model in which most major cardiac events occur is microscopic ruptures of vulnerable atherosclerotic plaques. However, most vulnerable plaques appear angio-graphically insignificant before rupture. Thus, the ability of stress testing of any type to detect vulnerable atheroscletotic lesions is limited by the lesser effect of these lesions on coronary blood flow and may explain the acute coronary events that occasionally occur after a negative treadmill test. Stress treadmill tests Individuals at risk for coronary heart disease are monitored for blood pressure, heart rate, chest pain, and electrocardiogram (EKG) while increasing oxygen demands on the heart, such as running on a treadmill. Nuclear medicine imaging of the heart can be added to increase the sensitivity and specificity of the lest. Individuals who cannot run on the treadmill (such as those with severe arthritis), may be given medications such as Persantine to stress the heart. In the case of diagnosing a patient with atherosclerotic coronary artery disease (CAD), one test that is frequently used is the exercise treadmill test. Exercise echocardiography improves upon the sensitivity and specificity of stress echocardiography by allowing for rest and stress imaging of the myocardium. The presence of a stress-induced wall motion abnormality is consistent with a highly stenotic coronary lesion. Stress testing allows for assessment of exercise capacity as well as stress-induced ischemia (chest pain or electrocardiographic changes consistent with ischemia) with a sensitivity of approximately 70 . Stress echocardiography, by visualizing the wall during rest and increasing levels of stress, increases the sensitivity and specificity of treadmill tesing to approximately 8590 . In comparison to the cycle ergometer, the treadmill increases the potential to recruit sufficient muscle mass to elicit a maximal metabolic response, particularly in deconditioned populations (Rowell, 1974). With bike ergometry 85-90 of the VO2max achieved with treadmill testing can be attained while only 70 can be achieved with either arm or supine ergometry (Rowell, 1974).
