Diagnosis of restless legs syndrome and periodic limb movement disorder

Due to the nocturnal worsening of symptoms, RLS has its greatest impact on sleep. Sleep disturbance is the most frequent reason that patients with RLS seek medical help 3 . Patients with RLS may suffer from severely disrupted sleep, which mostly presents as initial insomnia. Polysomnography, although not routinely indicated for the evaluation of RLS, reveals an elevated number of periodic leg movements during sleep (PLMS) in 80-90 of RLS cases 4 . PLMS are a sleep-related phenomenon that manifest with periodic episodes of repetitive, stereotyped leg movements 5,6 . These periodic movements are characterized by the extension of the big toe in combination with flexion of the ankle, knee, and sometimes the hip. Most RLS patients also will demonstrate periodic leg movements during wakefulness (PLMW) during polysomnographic recordings while awake 6 . Fibromyalgia and rheumatoid diseases

Chronic medical conditions may also cause significant sleep disturbance and manifest clinically as either EDS or fatigue. Patients with fibromyalgia frequently characterize their sleep as being restless, light, and unrefreshing 63 . These patients often have an alpha-frequency EEG coincident with delta-frequency sleep, or alpha-delta sleep 64 . Alpha activity is characteristic of the EEG pattern seen during quiet wakefulness with the eyes closed and does not typically occur during deep sleep (when delta activity occurs) in normal subjects. This EEG finding has also been reported to occur in rheumatoid arthritis and chronic fatigue syndrome 65,66 . Researchers have found a positive correlation between the frequency of alpha-delta sleep and severity of overnight pain in patients with fibromyalgia, and an inverse correlation between frequency of alpha-delta sleep and subjective sleep depth and or refreshing sleep 67,68 .

Respiratory Cardiovascular and Motor Changes During Sleep in the Elderly

The significance of respiratory dysfunction associated with sleep in the otherwise normal elderly is not well understood (56). Several factors appear to be involved, including aging-related alterations in neural and chemical regulation of respiration (Chapter 17). Abnormal rhythm of the heartbeat cardiac arrhythmias (Chapter 20) and pulmonary hypertension are common during apneic periods (57). Apnea, arrhythmia, and hypertension are usually most frequent during REM sleep they have been related to an increased release of norepinephrine from sympathetic stimulation, with mental disorders such as depression, or with the use of some neuro-tropic medications (Chapter 22).

Treatment and Outcome

No specific treatment is available for Gulf War syndrome. Treatment is symptomatic and similar to that prescribed for fibromyalgia. Low doses of antidepressants are used to improve sleep and decrease fatigue. Clinicians often prescribe cognitive behavioral therapy and exercise.
