Levels of norepinephrine as seen with aerobic exercise may increase platelet reactivity and hypercoagulability as well.27 Hypertension is also associated with increased physiological reactivity in the sympathetic and HPA axis. Studies indicate that individuals at higher risk for hypertension show increased cortisol, blood pressure, and heart rate in response to a psychological stressor.28 A study with nondiabetic individuals with acute myocardial infarction has also correlated increased cortisol and glucagon with insulin resistance.29
Current concepts to increase physical activity in patients with obesity and type 2 diabetes have shown only small efficiency. As most patients are completely sedentary or immobilized due to other health problems such as osteoarthritis regular physical activity is difficult to be established on a regular basis. There is no doubt that most obese type 2 diabetic subjects would benefit from at least some level of physical activity. As almost all of these patients have low physical fitness as assessed by VO2max only low-intensity training is possible and appropriate. Among the low-intensity activities, which should be recommended are walking at a self-selected speed, swimming, or gymnastics aerobics. There are sufficient data available to demonstrate an improvement of insulin resistance by low-to-moderate physical activity in men and women with and without type 2 diabetes (44). Chronic overuse injuries and accumulated minor and major trauma to the musculoskeletal system have a cumulative effect on the High-Mileage SEAL approaching the age of 40. The most common chronic musculoskeletal injuries involve the neck, back, elbow, knee, ankle and foot. Table 14-1 outlines the sites and causes of these various chronic conditions. It also presents selected recommendations for treating these conditions. Poor flexibility can be a major contributory factor in all of these conditions. A lifetime program of flexibility, strengthening, and cross-training for aerobic conditioning is essential for minimizing chronic injuries and pain. In the event of an injury, early self-referral for evaluation is an essential part of avoiding chronic injury.
A PAPAW can provide mobility similar to a manual wheelchair with less stress on the arm joints, while at the same time offering some of the aerobic exercise benefits of manual wheelchair propulsion (Arva et al., 2001). PAPAWs have been shown to reduce the amount of force required for propulsion, to lower the stroke frequency, and to reduce the range of motion required for efficient propulsion (Corfman et al., 2003a). There are also clients who desire a manual wheelchair, but who have difficulty negotiating ramps, side slopes, soft surfaces (e.g., carpet) and other mobility barriers. These individuals may have impaired upper extremities or their limitation could be due to disease or injury-related reduction in cardiorespiratory capacity. It may be difficult for

Reduced muscle mass and deconditioning are common among older patients, but there is substantial evidence that exercise can be beneficial in enhancing insulin sensitivity and improving glycemic control (49). Resistance training may be more effective in this regard than aerobic exercise (50,51) and may also be easier to implement for patients with orthopedic limitations, such as arthritis. Because of the high frequency of CVD in elderly patients with diabetes, cardiac evaluation should be considered before institution of any vigorous exercise program.
I truly believe that Qigong has helped me tremendously. Aqua-fit exercise, aerobic exercise performed in water, can be an alternative way to exercise in water. This can be a good method for patients with lower limb arthritis and for obese subjects. Endurance training includes aerobic activities such as walking, jogging, running, cycling, and swimming. Regular aerobic exercise is generally believed to raise the pain threshold by stimulating the release of endogenous opioids (4). In addition, the weight loss associated with these activities has been shown to reduce the severity ofjoint pain significantly. Swimming and pool exercises cause less joint stress and, when done in a heated pool, may actually provide analgesia. For an aerobic exercise program to be effective, however, it must be well tolerated. The activity must use large muscle groups, incorporate repetitive muscle contractions, and elevate the resting heart rate to the target heart rate for at least 20 minutes (5). Target intensity in the geriatric patient has been effective at 40 of maximum (5).
Widespread pain and tenderness are the cardinal features of FM. Stiffness after being in one position for a prolonged period, including morning stiffness, is common. Frequently weather changes, emotional stress, unaccustomed physical activity, and menses worsen the symptoms. Although patients often report a sensation of swelling, there is usually no objective evidence of this on examination. People with FM may be more sensitive to pain throughout the body, not just in the areas of tender points. For example, it is common to see increased visceral pain in patients with FM, so associated syndromes and symptoms, such as irritable bowel syndrome, dysmenorrhea, mitral valve prolapse, interstitial cystitis, and migraine headaches are more common.1 One must also be aware that tender points are common sites for pain even in the general population, who have an average of 3.7 positive tender points.4 Pain perception is also influenced by a number of other factors including aerobic fitness,...
